Patient Registration
You may preregister with our office by filling out our secure online Patient Registration Form. After you have completed the form, please make sure to press the Complete and Send button at the bottom to automatically send us your information. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.
COVID-19 OFFICE POLICY:
At this time we are limiting extra companions during your visits to the office – spouses and children will not be allowed in the office during your visit. Please make arrangements for this as we need to limit the number of people in the office at any one time. Companions are welcome to wait for you in your car during your visit. Only one guardian per minor will be allowed in the office.
If you have a temperature greater than 100.4 at the time of your appointment, or if you answer yes to any of the questions on your Covid-19 form, your appointment will be rescheduled to a later date when it is safe for you to be seen. This is for the safety of our team and other patients. Please contact us with any questions.
